Accelerated Three Year Program Request Form PDF | Word To be used by a first-year student to declare intent to graduate in three years. Completed form to be turned in to the Dean of Academic Services.
Add/Drop Form PDF | Word To be used during the first few weeks of the term to drop or add a course to a previously registered schedule. The form must be returned to the Registrar's Office according to published deadlines, and may require an advisor's signature.
Arts Management Certificate Declaration Form PDF To be completed by students who want to apply for the certificate program.
Change in Address Form PDF | Word To be completed by the student and turned in to one of the following offices: Registrar, Student Affairs, Business.
Course Block Distribution Form PDF Used as an aid to scheduling classes.
Diploma Information Form PDF | Word To be filled out by potentially graduating students with correct information to be included on diplomas and in commencement programs.
Enrollment Verification Form PDF | Word Request form to be completed by student if verification of enrollment is needed for insurance or other purposes. Please bring the completed, signed form to the Registrar's Office.
ESP Perspectives Course Approval Form PDF | Word To be used by faculty to apply for one of their courses to meet a general education perspective requirement.
ESP Skills Course Approval Form PDF | Word To be used by faculty to apply for one of their courses to meet a general education skill requirement.
ESP Tracking Form PDF | Word Used to track a student's progress toward meeting general education requirements (skills and perspectives).
Grade Change Form PDF | Word Used by faculty to change an incomplete to a grade, or to correct a previously recorded grade.
Graduate Request for Incomplete Grade PDF | Word To be used by graduate students if unable to complete a course by the end of the term, allowing the student an extension for one term to complete all coursework.
Incomplete Request Form PDF | Word To be used by undergraduate students if unable to complete a course by the end of the term, allowing the student an extension for one term to complete all coursework.
Independent Study Application PDF | Word This form must be completed and turned in to the Registrar's Office before a student can be registered for independent study credit.
Interdisciplinary Major Declaration Form PDF | Word To be completed by an undergraduate student, signed by advisors and the program director, and returned to the Registrar's Office.
Internship Agreement Form for Regular Terms PDF | Word This form is to be used when setting up and registering for internship credit in the fall, spring, and summer terms.

Major Declaration Form PDF | Word To be completed by the student and signed by advisors when declaring or changing a major, and turned in to the Registrar's Office.

Pass-Fail/Audit Form PDF | Word To be completed by the student, signed by the instructor and an advisor, and turned in to the Registrar's Office.

Replacement Diploma Request PDF To be used by Hollins alumnae to order a new diploma replacing one that has been lost or damaged.
Request for Approval of Courses to be Taken at Hollins-Affiliated Study Abroad Programs PDF To be completed by a student prior to attending a Hollins-affiliated study abroad program and turned in to the International Studies Office.
Request for Approval of Courses to be Taken at Other Colleges/Universities PDF | Word To be completed by a student who wishes to take additional coursework at a different college or university (e.g., at home over the summer or semester abroad through another school). Signatures of an advisor and the Registrar are required.
Roanoke College Reciprocal Agreement Form PDF | Word Form to be used by a student requesting persmission to take courses at Roanoke College during a regular term through the exchange agreement.
Summer Reading Application PDF | Word To be completed by a student who wishes to earn two credits for summer reading work.
Thesis Application PDF | Word To be completed by undergraduate students doing thesis work (honors or regular) in their senior year.
Transcript Request Form PDF Formal written request for release of the student's record in the form of an offical transcript. Must be signed by the student and returned to the Registrar's Office with $5.00 per copy.
